For an appointment
To schedule an appointment with a medical physiatrist, all prescriptions need to be sent by fax or by mail, TOGETHER with recent and pertinent test result reports. Please hand write clearly your name, address and telephone numbers on your request. If this is regarding a file with the CSST or SAAQ please note your client number with your agent`s name and your specific CSST office, for example (CSST Montreal or Laurentides) on your request. The reception will contact you in approxamately 1 to 2 weeks, after receiving your fax. Thank you
